Images loaded from wikitext always use the default 220px width, even if user's preference for thumbnail size if different (T69047).

However, if you make any change in the media dialog to such a thumbnail (e.g. modify the caption), it is immediately re-rendered with the user's preferred thumbnail size. This is confusing.
