The extension currently lacks the ability to declare the issuer name when setting up 2FA (see Google Authenticator's docs on this). We should add the ?issuer= parameter to the otpauth:// URL when a user sets up 2FA so that the user can easily differentiate it from other 2FAs enabled by the user.
