Well, that's fairly odd. The actual file is a 100kb JPEG image:

I don't see a notice in the upload log either (http://en.wikipedia.org/wiki/Wikipedia:Upload_log_archive/September_2004_(1)) however
it's possible a database hiccup caused the database updates to fail while the actual file upload went through. Odd to upload a JPEG as
a .png file though.

Also, both listed images appear archived, which seems consistent with an upload through the wiki without the database being
updated:

Last update was four years ago... assuming this is either an ancient hiccup or an ancient bug since fixed. Alternately, reopen if new instances surface.
