[quarry] Add 'feedback' link to pre-filled phabricator task

Description

A simple "Feedback" link in the main page that redirects to a pre-filled phabricator task with the quarry tag, and maybe some pre-filled title (something like "[feedback] <your subject here>") so we can know later they come from there.
