Regarding the task description above, I'm not sure "Add your username below" will work well on Phab?

Okay! It was not immediately clear to me what is possible w/ custom form and how it is different from a pre-filled form. That's why I ended up creating this task. I had forgotten about the tool Phabulous and the url with parameters that you generated is all what I needed.
Regarding the task description above, I'm not sure "Add your username below" will work well on Phab?
I think this would work. We need folks to add their Phabricator usernames below this bullet point like we have done in the past for gathering interest on sessions.
