Reuploading files with warning uploads new files

Description

Hi. I upload new versions of files many times, but yesterday it was very wrong. New file created without any sence or license or naming.

  1. Open Commons.
  2. Find two existing files, X and Y, with the same extension (.svg in my case).
  3. Download X to your computer (File:Moscow metro map sb.svg in my case).
  4. Change the downloaded file name, for example to "Phabtest.<same extension>" (Metromap.svg in my case).
  5. Upload it as a new version of Y (File:Moscow metro map sb draft.svg in my case).
  6. Get a warning "You're trying to upload a copy of existing file".
  7. Expected: Click on "Ignore warnings" and the new version uploads.
  8. Got: A new file, "File:Phabtest.<same extension>" created, with one version and empty license (File:Metromap.svg in my case).

I saw this a couple of times yesterday. Creating a file with local computer name is bad enough, not talking about creating an irrelevant file. Thank you.

Maybe it was just one day glich. I do not want to try today again, and then ask again to delete the wrong file.

Well, now I'm sure. It continues to happen.

Upload it as a new version of Y

Based on the tag, i'm assuming upload with UploadWizard.

I'm not sure how this is possible. If I upload the same file, with only the file name changed, the UW tells me "This file is a duplicate" and won't let me continue the process (this is actually an outstanding issue that people would like to see fixed in UW if i remember correctly).
