The bug is back today.

Toolbar seems to be back for me now, don't have time to test it rigorously but it showed up on the first few articles I tried looking at a few minutes ago.

In an attempt to quantify the degree of inconsistency, I just went through a day's worth of redirects in the backlog in Firefox, approving them using the [mark page as reviewed button]. I think less than 1/10 displayed no button or toolbar, about 1/10 displayed the toolbar correctly, and the rest displayed just the button.

Not sure if this is related to the above bug, but there used to be a "Refresh" button at the bottom of Special:NewPagesFeed, and now that's gone.

I think I've come across another instance of this bug occurring in the redirects created on November 26.

@AVasanth_WMF, my experience has been the same as what RoySmith describes above.

I tried to access Rock's Backpages and I got an error page saying that I don't have access to this resource. I can still log in just fine the old-fashioned way, however.

I'd say that on a typical day of patrolling the back end of the queue, I'll go through 150-300 articles, send 5-10 to RfD, tag around 5 with G5 or R3, and either retarget or convert-to-dab 5 more. Attack redirects are less frequent, I'll come across a handful of attack redirects per week.
