When both clean changes and RC patrol are enabled CC is removing the rcid parameter from the new pages with multiple edits in the same day (in enhanced RC, the link with the rcid is the first timestamp for the page), which makes it impossible to patrol them.
I've verified that it works on Meta, so it must be CleanChanges.

I'm not familiar with the extension not with the RC mechanism in general, and I don't have time to investigate this now. Sorry.
For the record, Hoo has a patch to overhaul the patrolling system, killing the rcid parameters for a good start: I1e24733c; it fixes a couple of other bugs, it might fix this one, too. It just needs someone "who's firm with memcached and DB performance" to look at.
Tim Starling finally merged Hoo's patch for bug 15936, so this bug is now superseded: I just patrolled my first page without rcid. \o/
