Well, it was never fixed. I don't know what, "should now be fixed" as it doesn't give how it was fixed. I see this 3-4 times a day.

Well, it was never fixed.
No, the bug as reported elsewhere was fixed (paste of a URL would be no-wiki'ed if it was input a reference, but not if pasted into the article). This is also the bug exhibited in all three of your examples.
However, you're right, there are a few minor situations where it is possible for users to frustrate the system and insert <nowiki>s anyway – specifically, when the content being pasted is longer than 80 characters (for performance reasons) or has multiple things in it, rather than just a simple link. I'm not sure there's a sane fix that doesn't damage/break the system for other users, however.
