Review QuickSurveys survey position & load order

Description

Survey positioning is inconsistent across pages. Sometimes it's above the ToC (https://en.wikipedia.org/wiki/United_States_Air_Force?quicksurvey=true), sometimes it's below (https://en.wikipedia.org/wiki/List_of_closed_railway_stations_in_Britain:_S), and sometimes you don't even notice it exists because the page takes too long to load (https://en.wikipedia.org/wiki/Barack_Obama?quicksurvey=true).

We should have a more consistent location for the survey and determine if it should really load after all content is loaded or not, given that many people appear to be scrolling down before it's been loaded. (A floating survey that scrolls with you could be one solution to this.)

Let's wait till a survey specifically needs it and fix it as part of that surveys deployment.
