Which wikitech pages would this form be linked from?
Where are such access permission requests currently handled?

@Aklapper @mmodell There's no formal on-wiki page where permissions are currently requested. All permissions requests I am aware of have been handled via Phabricator: T158799, T158315, T158489 & T151893 are the most recent examples.

With T158417 having been fixed, we're slowly filling those pages with content, such as https://wikitech.wikimedia.org/wiki/Wikitech:Content_administrators and much more to come, thus having a form would be easier and appropriate.

There's no jumping if wiki pages were never used as far as I am aware.

If possible, place Cloud-Services before wikitech.wikimedia.org (I know the order doesn't matter but looks cleaner that way).

Ah, the "(modifiable)" in the title was a note for the form creator that the field should be able to be editted. You can remove that too if possible. Thanks :)
