Upstream changes to CQLSSTableWriter

Description

During the course of T192689: Unchecked storage growth, we used a technique of bulk-importing SSTables to cull revisions from Cassandra. This however required some minor changes to the CQLSSTableWriter class to support DELETEs. This technique was useful, and probably will be again; We should upstream these changes so that they are available in future versions of Cassandra.
