And another bug, please see category box. I use Hotcat (see preferences) and there are a lot of buttons after a single reply (see image).
OOUI icons are not loaded when viewing pages, only when some part of the interface uses them. DiscussionTools uses this icon, so it was available after using DiscussionTools, but it won't show up normally.
I assume you got the idea from my post on "Talk:Talk pages project/Replying", where it worked for the same reason (Flow uses this icon), but it's a bad idea to use it there too, so I fixed it: https://www.mediawiki.org/w/index.php?title=Topic:W3vfn60hwy7jhld7&action=compare-post-revisions&topic_newRevision=w3ybmu5xld965hva

Seems like a bug in HotCat.
This has been recently reported at: https://en.wikipedia.org/wiki/Wikipedia_talk:HotCat#Doubled_HotCat_insertions_with_Reply_Tool
The thing with icons is not supported.
As a side note, the HotCat bug has been fixed.
