This was caused by a full disk.
I distinctly remember deleting the 32GB revisions.sql file after restoring the revisions table, but either I did something wrong and didn't actually delete it, or it somehow got restored.
Having that file around at the same time that the Dashboard tried to run its weekly database backup script resulted in a full disk.
