This feature seems to be exactly what we want—does it work on dewiki? "Endorse" button on https://meta.wikimedia.org/wiki/Meta:Probox/Default
We can also look at https://meta.wikimedia.org/wiki/Meta:FormWizard for a more complex form layout.

"Probox" turns out to rely on https://meta.wikimedia.org/wiki/MediaWiki:Gadget-addMe.js . Of course, if we can get a gadget deployed to dewiki we can have all the fancy UI we want... but this was already ruled out for this year.
On the Meta: page it says "It can be used in conjunction with the AddMe gadget and Form Wizard gadget, or on its own." But I'm not sure, what the functionality downsides are when used without these gadgets. - Also: Isn't anything with gadgets out anyways due to nojs users?
