Setting project to Wikimedia-Site-requests, as this request is about settings / configuration of a Wikimedia website and not about the MultimediaViewer codebase.

It isn't installed at all private wikis. I'll create a patch to enable it at officewiki if it is wanted.

Does MultimediaViewer work correctly with thumb.php/thumb_handler.php?

That could be a reason it's not enabled

It was disabled in https://gerrit.wikimedia.org/r/#/c/145612/ . At the time it was still available as a beta feature, but that disappeared when it came out of beta. The bug was probably fixed since (there is a private vagrant role and MMV worked correctly with it at some point).
