Currently, few of the OOUI elements are supported in the FormWizard. Therefore, there is need for more elements which are found here

I cannot even find any extension codebase. https://phabricator.wikimedia.org/project/profile/1496/ links to https://meta.wikimedia.org/wiki/Meta:FormWizard#Deployments but that's only the gadgets, and https://phabricator.wikimedia.org/T198769 links to no code base either. :(

@Jayprakash12345 We are working on moving the code to Gerrit soon. The project is on github at the moment.
