I'm not sure how maintained commonsarchive is these days, or who could look up the stacktrace for that error on that instance...

The "Leave feedback" link at https://commonsarchive.wmflabs.org/wiki/Special:UploadWizard points to https://phabricator.wikimedia.org/maniphest/task/edit/form/1/?projects=MediaWiki-extensions-UploadWizard

Viewing the page requires one to be logged in.

Error message reminds me of T94562, T187491 but in any case, very first thing that should happen before investigating anything here would be to have a maintainer of commonsarchive.wmflabs.org make commonsarchive.wmflabs.org run recent, secure software versions. That website is currently running ancient unsupported software versions, see https://commonsarchive.wmflabs.org/wiki/Special:Version . It also means that website is using a version of UploadWizard which at that time still had the "Leave feedback" link that you described. It has been removed in newer versions.
