Migrate Nuke's deletion confirmation form to Codex

Description

The Nuke deletion form is generated via old style HTML with the XML class. We can update this straight to Codex.

Proposed design:

Indented checkboxes_.png (932×1 px, 94 KB)

This uses the 'intermediate' state of checkboxes. When Select All is selected it should select all checkboxes below. If it is deselected all boxes below should be deselected. If the checkboxes are in a partial state of selection, the intermediate state should be used.
