Modify the user group application form on Meta

Description

As part of the Wikimedia Foundation's Affiliates Strategy process, the process for new application of Wikimedia user groups has been modified.

The ask here is to revise the Meta form currently used by AffCom to receive applications of new user groups (https://meta.wikimedia.org/wiki/Special:Contact/affcomusergroup).

@RamzyM-WMF Can you add more info about what exactly to modify? For instance to add additional question or a particular checkbox. It's not currently clear what to do from the request

Here are the mockup of the changes that I can Identify from that document. Please compare the images and let me know if there any more thing to do.

There's a textual description of the changes here on Meta-Wiki. You can edit that if it's not correct.

Note: Elements are not in the order as described in the document, I assume that's not a requirement.

Yes, this looks like what we wanted. A small ask: would you be able to arrange the elements in the order as described here? https://meta.wikimedia.org/wiki/Wikimedia_Foundation_Affiliates_Strategy/Implementation/User_Group_recognition_process#Updated_application_form

@RamzyM-WMF, Yes, they can be arranged in that order, see below. This requires some adjustment in MediaWiki-extensions-ContactPage which I will do.

Can you also say which fields are compulsory or optional to answer? In the above mockup all questions are compulsory

This includes this message:

If your group is planned to be an international group, you can put "International".

Will the word "International" be written manually, or can the user select it from some menu?

@Amire80 yes, it would need to be entered manually.

@RamzyM-WMF I am still looking for reviewers for the change 1082162 to move this forward.

Otherwise this is what is possible now, it cannot meet the requirements though. But I want to check if you okay with it this way.

@Ammarpad @Varnent @Amire80 hi all, we've had some applications after the form update, which is converted into an email through the user group recognitions mailing list. However, we did not receive anything from the "Letter of intent" element. Would you be able to check if applicants are able to fill out this one, or if there's any bug on that particular element? Hi @RamzyM-WMF, I believe you saw 'Logo' field repeated twice, the first one is actually the letter of intent link.

I have uploaded a patch to fix that and make it even more clearer.
