MediaWiki 1.16 has been long EOL'd, Wikia has migrated away from it recently, and the code of AbuseFilter and the edit page has changed a lot since then. Also the report doesn't contain anything that could be used to infer when/why this was happening. Probably best to close (feel free to reopen if this is still happning in current MediaWiki, or still relevant to Wikia).
