In bug 36577, we discovered that Varnish doesn't currently do well with streaming large files.
Tim wrote in an email conversation on this subject:
We could probably patch MediaWiki to use a different upload hostname
for large files. The image row should already be loaded from the cache
or database in all the relevant places, so there shouldn't be any
overhead. Then we could use Squid or some specialized media streaming
proxy for large files.
...to which Asher replied:
This sounds like a good idea even if varnish becomes the right solution for
all of our caching needs. We may want to select dIfferent storage for video
caches, and we should be able to cache videos above swift without
potentially evicting many images.
This bug is about implementing this idea.
