Template: Decision Statement Overview

Why: what is the value? Does it align with our broader values?
What: What is needed? What is in Scope and What is out of Scope?
Who: Who needs to be consulted? Who will be implementing this? Affected by this? What is the impact? (Using RACI format)
When? Any timeline?
Model or Diagrams (optional): A visualization to help distill the decision to be made.
