The Account Creator approach is probably simplest.
Another person ran into this at an editathon (specifically, "clicking "show options" -- it showed an additional trashcan icon; I clicked it.", I believe).
It is suggesting that there be no mw.loader.using calls in the main script (https://phabricator.wikimedia.org/diffusion/1884/browse/master/proveit.js), and where to put them in general.
For consideration, VisualEditor uses special numeric ones for this purpose (e.g. <ref name=":0">).
It would also be useful to have form fields (the UI) to and from the object.
